Free Consultation

A short conversation to see if we're a good fit. No pressure, no commitment.

1. Your Details

Required — just so I can reply to you.

So I know who I'm talking to.
Where I'll send my reply. Double-check the spelling.
2. Your Goal

What you want out of coaching. Pick at least one.

Tap every goal that applies. Pick “Other” if none fit and explain in section 5.
3. Your Baseline

A quick snapshot of where you are today. Rough guesses are fine — fill in at least one.

Your current weight. BF% means body fat percentage — only add it if you know it.
Roughly how many hours you sleep on a normal night.
About how much water you drink a day. 1 L is roughly 4 cups.
1 = very relaxed, 10 = completely overwhelmed.
What a normal day looks like — sitting at a desk, on your feet, physical work.
4. Training & Commitment

How you like to train and how much time you have. Answer at least one.

Beginner: little or no regular exercise. Intermediate: training on and off for a while. Advanced: training consistently for years.
Lifting = weights or resistance. Cardio = walking, running, cycling. Mix = some of both.
How many days a week you can realistically exercise.
How long each workout can be.
Where you'd work out — a gym, at home, or both.
5. The Real Why

The part that matters most. Answer at least one — a sentence is enough.

The real reason you want to change. A sentence or two is plenty.
Anything that has gotten in the way before — time, injuries, motivation, food.
How sure you are that you can stick with it. 1 = not at all, 10 = completely.
Foods you can't or won't eat, plus anything you crave often.

This form goes directly to my inbox. I'll reply within 24 hours.

Full Client Intake

For serious applicants. This gives me everything I need to build your first month.

1. Personal Information

Name and email are required; the rest helps me reach you and keep you safe.

Your first and last name.
Either your birthday or just your age.
Where I'll send my reply. Double-check the spelling.
In case it's easier to reach you by call or text.
Your job, or what fills most of your day.
Someone to call if something happens during a session.
2. Health History (PAR-Q+)

Please answer all four Yes/No questions. This is a standard pre-exercise safety check.

These standard safety questions tell me whether to check with your doctor first. A “Yes” doesn't rule you out.
Has a doctor ever said you have a heart condition?
Do you feel chest pain during activity?
Do you lose balance or consciousness?
Any bone/joint issue worsened by activity?
Anything you take regularly, including vitamins or protein powder. Write “None” if nothing.
Food or medication allergies. Write “None” if nothing.
Your family doctor, if you have one.
Your doctor's office number.
3. Purpose & Motivation

Tell me what you're working toward. Answer at least one.

What you want to be different — how you look, feel, or what you can do.
What made you decide to start now.
What happens if nothing changes in a year? Be honest.
4. Baseline Metrics

Your starting numbers so we can track progress. Height and weight are a great start — fill in at least one.

Take these in the morning, fasted, after using the bathroom.

e.g., 5'6" or 168 cm.
Your current weight in lbs or kg.
Tape measure just below the Adam's apple / voice box.
Tape around your belly, level with the belly button.
Around the widest part of your hips and backside.
Around the fullest part of your chest, under the arms.
Around the thickest part of the upper arm, relaxed.
Around the thickest part of the upper leg.
Only if you know it, e.g. from a smart scale. Leave blank otherwise.
5. 3-Day Dietary Log

Write down what you normally eat. Even one day helps — add at least one food.

Log everything for 3 consecutive days. Be precise with quantities and brands.

Day 1 food log
Food Item (include brand)Qty / Time
Day 2 food log
Day 2 — Food ItemQty / Time
Day 3 food log
Day 3 — Food ItemQty / Time
